Our mission
CoviCare24 is an initiative of the Primary Care Service (SMPR) of the Geneva University Hospitals (Faculty of Medicine, University of Geneva) aiming to facilitate the dissemination and sharing of COVID-19 processes for ambulatory care centers worldwide. Ambulatory care centers, especially those operating 24/7, must be able to meet the needs of patients and the medical community during the COVID-19 pandemic. Our goal is to help other centers coordinate the response between different medical and social partners in the current crisis in order to provide excellent care without exception. An ambulatory care center must be able to meet the specific medical, social and mental health needs of patients suffering from COVID-19 and their families.
The SMPR of the Geneva University Hospitals along with other ambulatory care centers had to, within a few weeks, deal with the exponential COVID-19 pandemic. Our team has implemented, in collaboration with local partners, strategies (processes, guidelines) and structures (a 12-place discharge area, a telemedicine and follow-up center, an ambulatory pneumonia monitoring unit for COVID-19 , etc.) to ensure the application of best practices in this situation. The general organization is based on the CoviCare concept of the canton of Geneva. CoviCare24 is a platform aiming to make these different processes available but decontextualized to facilitate their use elsewhere by the medical community and more particularly by those in charge of ambulatory care centers worldwide.
Our approch
Our main approach is based on the CoviCare concept of the canton of Geneva and consists in ensuring tailored follow-up for patients who are at risk of having COVID-19 or who have an established diagnosis. Regardless of the testing strategy (RT-PCR), we ensure a proactive follow-up by contacting patients every 48 hours and assessing their needs on a medical, social and mental health levels in order to make a shared and well-informed decision on the next steps to undertake. Patients also have the option of an electronic questionnaire-based follow-up via a patient-reported outcome (at SMPR PRO-COVID) if they prefer, with automatic alerts to the medical team in case of new or worsening symptoms.
Patients can be immediately contacted by an innovative and secure telemedicine system (at SMPR HUG @ home), which helps the medical team assess patients’ needs and decide on follow-up. A patient can then benefit from a home visit by a team of specialized doctors, a follow-up in the ambulatory care center for further investigations, closer monitoring or a hospitalization. We wish to actively share (Free Access) our processes with all healthcare professionals, hoping to help them in their response to the COVID-19 pandemic.
